On 25th May 2018, General Data Protection Regulation (GDPR) legislation came into effect. GDPR replaces the previous data protection legislation, the Data Protection Act, 1998.
Personal Data I will collect
Name, address, telephone number, email address.
- Next of kin details (should you become unwell whilst we are working together).
- Membership body, insurance company, ICO subscription
- Student Counsellor/Employed/Private Practice.
How I obtain this information and why I have it?
Any information I process is provided by yourself either in person or by you using electronic means
- The information I collect is based on the requirements of my insurance company and their requirement for me to collect personal identifiers, consultation and supervision notes.
How I store this information
All information is stored on paper in a secured filing cabinet.
- All emails are printed and stored in your personal file.
- After our work together is completed, any paperwork will be stored for the period specified by my insurance company.
Your Data Protection Rights
Under GDPR you have rights as an individual which you can exercise in relation to the information, I hold about you. GDPR requires that I, Kathy Lock, obtain consent to hold personal information about the supervisees with whom I am working. The information you completed during our first contact on the Supervisee Intake Form is held on paper and is kept in secure locked storage. Notes will be kept for a period of 5 years after supervision has ended in line with my insurance requirements.
You have the right to request to see your private file whilst we are working together. You also have the right to request to see your private file when our supervisory relationship has ended and I will respond to any request from you within 20 days.
You have the right to ask for information about you to be erased as long as it is no longer relevant to the original purpose it was collected. You can also request any inaccurate information about you be amended.
Should you email or text me, I will delete these messages after reading, and if relevant, any saved emails will be kept on hard copy and stored securely. I will keep your email and telephone details for 3 months after supervision has ended or when I last heard from you.
If you wish to withdraw your consent to my use of your data at any point, please let me know in writing by letter or email, however data may need to be retained for lawful purposes in line with my insurance requirements in case it may become necessary to share your data in the case of legal action following a complaint, or a legal duty by court order.
